Population Health Management
Tejas works closely with our community-based organizations, providers, and health plans to develop new and innovative programs to offer the most effective interventions for complex populations with high-risk health care needs, while reducing the overall cost of the patient’s care.
Our providers have a depth and breadth of experience working with individuals with long-term, chronic illnesses, both medical and behavioral health. We are experienced in caring for individuals with mental illness who also deal with co-morbidities such as diabetes, cancer, emphysema, chronic obstructive pulmonary disease, and other chronic, physical health issues. Our community-based, strengths-focused, recovery-oriented approach allows individuals who, historically, may have required much more costly institutional services to be safely and effectively cared for in their home community.
Education and support are essential in providing these individuals with the assistance they need to remain in or return to independent living as quickly as possible with less recidivism. We work directly with managed care organizations on discharge planning, post-discharge care coordination, and follow-up. We help our providers improve their skills and put their capabilities together to effectively intervene with difficult, complex populations. Our entire focus is on developing services and capabilities specifically geared toward reducing overall cost and improving patient care.
